District of Columbia, Department of Health Care Finance — Nursing Facility Quality Improvement Collaborative

The Medicaid Nursing Facility Quality Improvement Collaborative is a 5-year quality initiative that's based on the Institute for Health Care improvement’s Breakthrough Series model. Sponsored by the District of Columbia Department of Health Care Finance and led by Comagine Health, the initiative’s aim is to improve the quality and safety of resident care by engaging 17 local nursing homes in system-level, rapid-cycle improvements.

Collaborative members work together and individually to test system changes for improving quality, encouraging innovation, sharing successful practices and strategies, and building tools for participating in pay-for-performance programs.

  • Clinical topics related to quality measures
    • Pressure Ulcers
    • Antipsychotic Medication
    • Urinary Tract Infection
    • Bowel or Bladder Incontinence
    • Falls
  • Care Transitions
  • Readmissions
  • Interventions relevant to findings and gaps identified by the Consumer Assessment of Healthcare Providers and Systems surveys


Comagine Health is under contract with the District of Columbia Department of Health Care Finance to lead the Collaborative and work with partners such as the District of Columbia Health Care Association. We provide learning sessions and technical assistance to nursing facilities in the District of Columbia.
